I don't have a solid answer but I guess in case of IEEE there is a review
board. Each paper falls into a category like DSP or embedded systems. They
ask their in house experts to review the paper.

And quality. There are certain criteria like proper references,
significance of work etc. For example, If the paper describes already
existing cloud computing in length and simply concludes by saying that
cloud computing has a bright future without any comparative studies leading
to a new deduction, I believe that paper will get poor reviews.
But even if you do cutting edge research and does not follow proper
referencing format, that would be scorned upon as well. The paper should
also discuss in brief about previous work done on the subject and also why
they have to be improved.

I will dig deeper and come back with screenshots/links about the review
process later.
